A cap seal is required, and was missing, when the item is a liquid, paste, gel, or cream and all of the following hold [2]:
- Is 4.2 oz (125 mL) or more
- Is non-hazmat
- Is non-fragile
- Has a lid type of twist / screw top, clip-on top, flip top, or hand pump
- Does not have a secondary seal (see 6 for the pump exception)
✓ How to prevent it
- Tighten the lid before applying the cap seal [1]
- Twist / screw tops → apply the seal horizontally around the cap [1]
- Flip / clip tops → apply vertically over the opening [1]
- Pumps → apply vertically over the pump, attaching to the bottle on both sides [2]
- Items with multiple openings → a separate seal for each [1]

This is the only preparation type for which Amazon publishes an exemption. It splits by closure type [2]:

Cases where a cap seal is not the right prep at all (a different prep applies instead) [2]：
- Hazmat / dangerous-good liquid → bagging or heat shrink required instead
- Fragile → bubble wrap required instead (a cap seal may still also be needed if the wrap prevents breakage but not leakage)
- Spray bottle without locking mechanism → bagging required instead
- Fails a three-foot drop test → bagging or bubble wrap required instead
